The story of a supplier who misunderstood this requirement and signed the contract for 6,000,000 telephones to be designed and manufactured for a buyer. The supplier’s designed in expected life was found to be just 24 years as compared to the anticipated figure of 97 years. This design was rejected and reliability improvement programme was implemented. The result was an improved reliability of 76 years for this telephone was accepted by the buyer with a reduction in price. Thus the supplier suffered financial loss as a result of not understanding the requirements due to the lack of Reliability knowledge.
